National ID (PhilSys): Checklist
Name: [Your name]. To register, update or replace. The PhilSys website was under maintenance on 2 November 2026, and several steps come from PSA notices. Confirm yours at hotline 1388 or info@philsys.gov.ph.
1. What you need
☐ Not registered yet: your birth certificate (R.A. 11055). PSA’s last FAQ asks for a PSA birth certificate with one government ID, or a Philippine passport, UMID or LTO license.
☐ Wrong details: your PhilID or ePhilID and a supporting document.
☐ Damaged card: the card itself.
☐ Lost card or ePhilID: ask 1388 first. PSA’s FAQ says to wait for its notice on replacing lost cards, while regional notices ask for an Affidavit of Loss.
2. The form
– Updating and correcting are services at a registration center. You do not register again.
– Damaged or peeled card: ask for the replacement request form.
3. Your application
– Name, sex, date or place of birth: PSA or local civil registrar birth certificate.
– Civil status: PSA or local civil registrar marriage certificate.
– Address: barangay certificate or another document with your address.
4. Where to file
– In person at a PhilSys registration center. The law names PSA offices, local civil registrars, GSIS, SSS, PhilHealth, Pag-IBIG, COMELEC and PHLPost. Ask 1388 which one is open near you.
– Updates and replacements: PSA notices point to permanent centers. A damaged card is surrendered there.
5. Time limits
– Waiting for your card? Enter the 29-digit number from your Transaction Slip at tracking.phlpost.gov.ph.
– “Not Found” means the post office has not received your card yet. If it stays that way, call 1388.
6. Keep it
– Keep your Transaction Slip. A lost transaction number can be retrieved free at a registration center.
– Digital National ID: national-id.gov.ph or the eGovPH app.
– First issuance and renewal are free for citizens. The law allows a fee for a replacement card and PSA may waive it, so ask the amount.
– PSA warns against registering twice. Use the update service instead.
